IT’S EASY AND FAST TO GET STARTED USING LABORCHEX!
In order to begin using LABORCHEX services, HPBA Members must complete several forms. These include:
New Client Set Up Form (2 pages)
This basic fact-finding form provides basic information about the HPBA Member, including names, addresses, contacts, projected employment screening needs, expected method of ordering screenings and receiving results, etc.
Client Service Agreement (3 pages)
This agreement represents a formal agreement between LABORCHEX and the HPBA Member.
Electronic Access Agreement (1 page)
If the HPBA Member will be using the LABORCHEX website to order screenings and/or receive results, this form must be signed by both the representative of the Member and all persons designated as "End Users." The "End Users" are the actual persons who will be permitted to access company information via the LABORCHEX website in order to order employment screenings and receive results.
Investigation Authorization (also known as the "Release") (1 page)
All employment screening orders to LABORCHEX must be authorized by the job applicant. If a HPBA Member does not have an adequate Release (the statement that the applicant signs authorizing the screening) as part of their standard job application, they can use the generic release we provide (and just make the necessary number of copies). It can be found on our website, and HPBA Members can customize it by placing their letterhead on it. Note: HPBA Members using the website must "electronically acknowledge" that they have a copy of the applicant’s signed release on file…and this can be the release inherent in their standard job application or the LABORCHEX generic release. Any changes to the LABORCHEX release must be approved by LABORCHEX management.
Special Forms Required By States
Occasionally, individual states require that specific forms must be completed in order to acquire certain public records, such as driving and criminal records. These forms are accessible via the LABORCHEX website or by calling LABORCHEX. If a HPBA Member attempts to order a screening that requires such a form, they will be directed to further instructions.
Sample Adverse Action Letter You would forward a letter such as this to an applicant if employment is denied based on information (wholly or in part) contained in a LABORCHEX report.
Application Cover Sheet Many LABORCHEX clients use a version of this sheet as a cover for their standard job application. It serves as a notice to applicants that they might be screened, while also, hopefully, working as a deterrent to those persons who you would rather not deal with in any way.
